The following forms are designed to help us work together more easily:
If you are a new client and would like to save time for your first visit, please print off and fill out the "new client intake form" and "general liability release form" LOCATED BELOW in advance and bring them with you to your first massage session.

General Liability Release Form
This is a standard release of liability for our massage therapy work together.
This form allows a parent or legal guardian to give permission to perform massage on a minor client.
